I think the "no" box should be checked because it says "intrastate only" and obviously we drive outside of NYS. It's just the way that it is worded makes me unsure. They could have just wrote interstate or intrastate and we would check interstate. I also understand as you say that the doctor feels it out. I'm going to "Buddy" so I am just double checking him ahead of time.

Read your first post too quickly. (I read it as "should we check intrastate, yes or no.") Yes, the doctor fills it out, but if it asks "yes" or "no" to intrastate, you're correct, it would be "no".
