Has anyone else gotten the new Medical and Self Certification for Commercial drivers in the mail? i just want to make sure I'm doing it right. Any idea what box we're supposed to check? Thanks.

Received mine several mos. ago. The box I believe is N1 it is the one where you can drive interstate and you mail a COPY of our annual medical form. It is the choice without all the bs endorsements...unless you have a hazmat then you have to go to dmv. I hope this helps.

Yes, that's correct. NI and copy of medical certificate.
