Title: Important Info about your Medical Certificate and Physical

According to FMCSR, the medical certificate you get when you have your DOE physical can be dated 2 years from the date of the exam. You will still have to get a medical exam every year, but having your doctor date the medical certificate that you carry will help avoid future difficulties.

Also, as of May 21, your exam must be by a doctor who is certified by the USDOT (that's FEDERAL, not state). Aquafredda is so certified. If you go to another doctor make sure he/she is.

Last but not least ... You must submit an updated medical certificate 20 days before your old one expires to give the DMV time to update your records. You should mail this as soon as you get it. Maria in the office will also e-mail it to them to make doubly sure they get it, but it is still your responsibility to send it along with a copy of your license.

This leaves a small window of about 7-10 days during which to get your physical, because if you go more that 30 days before it is due, you will not be eligible to be reimbursed ($35) by our medical coverage. So, you want to be within 30 days of when it is due, but but go in time ... 20 days ... to have a new certificate before the old one expires.

If you get your medical certificate dated for 2 years, you will have more leeway as to when you go for your physical next year because your certificate won't be expiring at the same time your medical is due.

So far, Acquafredda is refusing to date the certificates for 2 years.

He's also saying that he's going to increase his fee. If so, we might be better off using a doctor who takes our insurance and just paying the $15 co-pay in the future.

You're better off using your own doctor, Just pay the Co pay and that's it. Also if you had heart bypass surgery make sure your Doctor puts that down on the form

The point is, as of May 21 the doctor you use must be FMCSA certified. If your doctor is not in the registry (link above) he's not certified and you can't use him for this purpose.

The list I provided above is of every doctor on Staten Island I could find on the registry. This is a new regulation and a lot of doctors may not yet be aware of it. I think in the future a lot more doctors will become so certified. (This involves training and a test.)

More than likely these doctors (except for Acquafredda) take our coverage and we'd still have to pay only the $15 co-pay.

I used a 10 mile radius of zip 10312 so nearby Jersey is intermingled with Staten Island. Two I would point out would be the Minute Clinic inside CVS on Hylan Blvd in Old Town and Ivy Urgent Care on Richmond Rd.

Note: A driver told me this morning that Heartland Medical is no longer in business.

Note: I'm told that Acquafredda now charges $75

you have to be very careful here just because a doctor takes our insurance blue cross,,denies payment on part of athe physical so if u just pay the co-payment you can  be billed later by the doctor...case in point heartland villiage medical...157.00 blue cross gives u the 40.00 and u also paid the 15.00 co-pay

You're right. Yet some people get away with it. Maybe the doctor puts it in as something other than yearly physical???

If my DOT card is dated to expire in 2016 - why do I need a medical this year?

I believe so, but not sure.

The only advantage to getting it dated for 2 years is so that the following year it gives you extra time for DMV to update their records and avoid decertification of your 19-A. You still have to get a medical every year to satisfy OPT (as per state law).

Yes, for a normal CDL it's every 2 years. But for school bus drivers it's every year (or more often for some medical conditions) as per state education department regulations ... which, I assume, is why OPT requires same.
