Some drivers are reporting that their tax preparers are requiring a form "1095-C" which is proof of health care coverage. If you need one you can request it ... call the union.

However, according to the IRS there are other forms of proof that are acceptable:

    Form 1095 information forms,
    insurance cards,
    explanation of benefits,
    statements from your insurer,
W-2 or payroll statements reflecting health insurance deductions,
records of advance payments of the premium tax credit and
other statements indicating that you, or a member of your family, had health care coverage.

If your tax guy is telling you he needs that form, tell him to check the law.

p.s. - The law makes it incumbent upon the EMPLOYER to send (give) a copy of the 1095-C to employees. However, since this is the first year it's required they have been given an extension to March 31 to do so.
