I think it maybe how the doctor bills it   What Doctor did you use    Aqua is listing the three things he does   Nothing wrong with how he fills out the form   Also he gives the complete medical and knows the form   That's why I use him for my work medical

I wasn't being facetious when I said he was the best at it. People use other doctors and sometimes have to take the form back to their doctor because he didn't fill in something or filled it in wrong. Aqua is a "specialist" in DOE medicals and he gets it right.

And don't worry, PROUD MEMBER, just because there are holes in the "oil cloth" and his blood pressure cuff is held together with safety pins, that doesn't mean he doesn't do everything he's supposed to do for the exam.

Day before or even same day ... early. He prefers a Monday or Tuesday so he can check your TB test before Friday, but he will come in on Friday too if he can/has to.

Well after all that I got a response from Blue Cross / Blue Shield after I sent in the Reimbursement Form . They sent a letter back with a Copy of the reimbursement form stating that "We can't Payout for a Date that hasn't occurred yet!" So I looked over the form and I was examined on 1/26/16 and Dr. Aquafredda wrote on the paper in 3 separate spots 11/26/16 in the "Dates of Service Section"! Lmao !! I called BCBS and they stated that stuff like this happens all the time with him ! So I asked if I could White out one of the 1's on each of the dates and the lady said "Yes!" I will keep you updated !

And the worst part is that it took them over 2 months for them to send it back to you!

Exactly ! Plus they only sent a Photocopy not the Original! We will see !

you can email and fax it to them also   and keep calling them...i had similar problem last year   i faxed the corrected one and kept calling bcbs every few days...
